A Y.E.S Support Coordinator collaborates with you to negotiate with providers about what supports they will deliver and the associated costs under your plan. They ensure service agreements and bookings are completed. They help you coordinate supports and access services, including any necessary legal or regulatory frameworks.

Preparing for Plan Reviews
Our Support Coordinators help you prepare for NDIS plan reviews, ensuring the right evidence and documentation are in place to support your ongoing needs.
We work with your support team to identify what is working well, where supports may need adjustment, and how to ensure your next plan continues to meet your goals.

Support coordinators will assist you to ‘optimise’ your plan ensuring that you are getting the most out of your funded supports.

Note: You need to ask the NDIS in your application or your Planner for Support Co-ordination if you think you will need it.

There are three levels of Support Co-ordination that can be included in your plan as described by the NDIS:

Support Connection

This support is to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.

Support Co-ordination

Co-ordination of supports: This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.

Specialist Support Co-ordination

This is a higher level of Support Co-ordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Co-ordination will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

This support will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
