Getting started with aged care services made simple

The new Support at Home program helps older Australians receive the right care to stay living safely and happily in their own homes.

Here’s how you can access funding and begin your journey with Y.E.S Aged Care.

Step 1: Check Eligibility

You must be 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people) and need assistance with daily activities . Eligibility is determined through an Aged Care Assessment.

Step 2. Contact My Aged Care

To receive government-funded home care, you’ll need to register with My Aged Care — the national gateway for aged care services.

You can do this by:

A trained assessor will:

  • Ask about your health, home life, and support needs
  • Determine what level of assistance you’re eligible for
  • Arrange an in-home or phone assessment (by the Regional Assessment Service or Aged Care Assessment Team)
  • A trained professional (nurse, social worker, or health care assessor) will visit you at home.
They will determine the level of support you need and whether you qualify for Support at Home (SaH).

Once approved, you’ll receive a Support at Home level (similar to the current Home Care Package levels) and can choose Your Enabled Services as your provider.

Step 3: Receive Your Approval Letter
  • After the assessment, you’ll receive a letter confirming your approved level of care (Level 1–8, with Level 8 being the highest need).

You will be placed on the national waiting list. Some of your aged care costs will depend on your financial circumstances. To find out how much you will pay for Support at Home, you will need an income or means (income + assets) assessment.

Step 4: Wait for an Allocation
  • The wait time varies, typically 3 - 12 months depending on demand.

You will receive a letter when your funding becomes available.

Step 5. Choose Y.E.S Aged Care as Your Provider

When you’re ready to start your services, simply tell My Aged Care that you’d like Your Enabled Services to manage your package.

We’ll guide you through every step of the setup and transition — including any transfer from an existing provider.

We can also help you with:

  • Understanding your funding and fees
  • Comparing management options (self-managed or fully managed)
  • Planning how to get the most from your budget
While you are waiting for your Support at Home funding to be approved, it’s a great time to start exploring and connecting with service providers.

Step 6: Choose a Home Care Provider

  • Once you receive your assigned package, you have 56 days to choose a provider (extensions are possible if needed). 28-day extension, giving you up to 84 days total.
  • Compare providers based on services, costs, and flexibility.

Sign an agreement with your chosen provider.

Step 7: Start Receiving Services

  • Your provider will create a care plan based on your needs and preferences.

The government pays your package funds to the provider, who manages your care services.

Step 8: Annual Reviews & Future Adjustments

  • Your needs may change, so regular reviews ensure your package continues to meet your requirements.
  • If you need more support, you can request a higher-level package.
