How to Access Support at Home

Getting started with aged care services made simple

The new Support at Home program helps older Australians receive the right care to stay living safely and happily in their own homes.

Here’s how you can access funding and begin your journey with Y.E.S Aged Care.

Step 1. Contact My Aged Care

To receive government-funded home care, you’ll need to register with My Aged Care — the national gateway for aged care services.

Once approved, you’ll receive a Support at Home level (similar to the current Home Care Package levels) and can choose Your Enabled Services as your provider.

Step 2. Choose Y.E.S Aged Care as Your Provider

When you’re ready to start your services, simply tell My Aged Care that you’d like Your Enabled Services to manage your package.

Once we receive your referral or enquiry, our team begins a simple, person-centred intake process to ensure your care is built around you.

Step 1: Welcome & Initial Contact

Our friendly Intake Officer or Care Coordinator will call you (or your nominee) to discuss your current supports, needs, and goals.

We’ll explain how the Support at Home system works and answer any questions about costs, services, and timeframes.

Step 2: Care Planning & Assessment

We’ll book a face-to-face or phone assessment to:

Step 3: Service Agreement & Budget Setup

We’ll prepare a clear, easy-to-read Service Agreement and show you exactly how your funding will be used.

Once you sign and approve your plan, services can begin — often within days.
