how to get Support at Home (SaH)

There are a few steps involved in setting up your Support at Home (SaH), and we will explain them to you in detail. We’re here to help you navigate through this process.

Step 1: Check Eligibility

You must be 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people) and need assistance with daily activi-ties. Eligibility is determined through an Aged Care Assessment.

Step 2: Apply for an Assessment

Contact My Aged Care at 1800 200 422 or visit www.myagedcare.gov.au

Step 3: Assessment

They will determine the level of support you need and whether you qualify for Support at Home (SaH)

Step 4: Receive Your Approval Letter

You will be placed on the national waiting list. Some of your aged care costs will depend on your financial circumstances. To find out how much you will pay for Support at Home, you will need an income or means (income + assets) assessment.

Step 5: Wait for an Allocation

You will receive a letter when your funding becomes available.

Step 6: Choose a Home Care Provider

Sign an agreement with your chosen provider.

Step 7: Start Receiving Services

The government pays your package funds to the provider, who manages your care services.

Step 8: Annual Reviews & Future Adjustments
