Wellbeing Support Plan
During our initial consultation, the information we collect becomes the basis for a personalised Wellbeing Support Plan. These plans identify the individual’s need-oriented goals that are met through deliverable strategies. Strategies are planned and worked on in conjunction with the entire Allied Health and Community team supporting our client.
Every Wellbeing Support Plan is customised to each client and can include:
