Onboarding consultation

Many of our clients exhibit complex behaviours related to their disability; so, when it comes to providing effective disability services, we really want to get to know those we serve.

Onboarding consultations allow us to observe and assess a client’s behaviour and daily living requirements. Identifying patterns, limitations and collecting information and getting an understanding on any client’s triggers will help us develop a wellness support plan for that individual. By building a plan that centre around what each of our client’s positively respond to, such as activities and preferences, we effectively reduce unwanted behaviours and allow our clients to have the control and choice on the life they wish to live.
The Process
With each new client, a Y.E.S specialist will conduct a thorough consultation. Time is spent with the client and other stakeholders in the client’s life, that are appropriate, like parents, caregivers, teachers and other Allied Health practitioners.
Our team is passionate about providing customised and highly personalised care to our clients, and in order to do that we really need to get to know our clients. When inducting and training our staff we stress the importance of empowerment, independence, inclusion and employment. Part of the onboarding consultation is to get an understanding of our client’s needs in these areas and incorporate them into the personalised Wellbeing Plans so that care can be consistent no matter what staff member turns up.
We can provide this service both in-person in our Cashmere office or at your home location if located in Brisbane, Gold and Sunshine Coast or if out-of-region via telephone/video conference.
Empowerment
People with disabilities have the same rights as everyone else. We work to educate our clients on their rights, the responsibilities associated with those rights, and the opportunities to exercise those rights.
Inclusion
Individuals must be included in their community through common interests, goal setting, and relationship building. We seek out community partners so that we can provide meaningful opportunities for our clients to realise their dreams .
Independence
Individuals should work towards becoming more independent through community participation. We help our clients identify areas where they would like to become more independent, and support them in taking steps to make this happen.
Employment
Employment is the key to breaking down social barriers that many Australian with disabilities face. The workplace can be the most social aspect in a person’s life. This is where they are able to feel pride and a sense of accomplishment by doing jobs that are just for them.
Everybody has the right to work, but this is not easily achieved by people with disabilities. Our team focuses on building bridges in the community for clients with disabilities to find work if this is a goal.
