how to get a home care package

There are a few steps to take a Home Care Package, and we will explain them to you in detail. You just need to follow these steps to navigate the process smoothly.

Step 1: Check Eligibility

You must be 65 years or older (or 50+ for Aboriginal and Torres Strait Islander people) and need assistance with daily activi-ties. Eligibility is determined through an Aged Care Assessment.

Step 2: Apply for an Assessment

  • They will ask questions about your health, lifestyle, and care needs.

If eligible, they will arrange a free Aged Care Assessment Team (ACAT) assessment.

Step 3: ACAT Assessment

  • A trained professional (nurse, social worker, or health care assessor) will visit you at home.

They will determine the level of support you need and whether you qualify for a Home Care Package (HCP).

Step 4: Receive Your Approval Letter

  • After the assessment, you’ll receive a letter confirming your approved level of Home Care Package (Level 1–4, with Level 4 being the highest need).

You will be placed on the national waiting list. Some of your aged care costs will depend on your financial circumstances. To find out how much you will pay for a Home Care Package you will need an income or means (income + assets) assessment.

Step 5: Wait for a Package Allocation

  • The wait time varies, typically 3 - 12 months depending on demand.

You will receive a letter when your funding becomes available.

Step 6: Choose a Home Care Provider

  • Once you receive your assigned package, you have 56 days to choose a provider (extensions are possible if needed).
  • Compare providers based on services, costs, and flexibility.

Sign an agreement with your chosen provider.

Step 7: Start Receiving Services

  • Your provider will create a care plan based on your needs and preferences.

The government pays your package funds to the provider, who manages your care ser-vices.

Step 8: Annual Reviews & Future Adjustments

  • Your needs may change, so regular reviews ensure your package continues to meet your requirements.
  • If you need more support, you can request a higher-level package.
