Supportive Care Model
Supporting all the needs of all individuals by providing engaged, innovative and quality care.
A Framework of Support
Historically, providing care to those with complex needs has been inflexible in nature, with behaviours being treated as things to correct and people with disabilities unable to connect with Allied Health partners or finding it difficult to participate in various community events.

At Your EnAbled Services (Y.E.S) supports we developed a staff training program and embedded a care plan framework that is supportive in nature, meaning that the behaviours of neurodiverse individuals are recognised, celebrated, and all people are empowered and can be involved in the community should they desire to be.

All staff will be vetted and trained to the highest standards and provided with the support and resources to ensure that our clients are safe in their care and supported 100% in their daily living environments.
Goals, Standards, and Wellbeing
We are not just supervising our clients, or acting like a babysitter. Our company is lucky to have the resources, staff with a wealth of experience and specialisation so that we can provide systematic and regular review to help our clients grow, evolve, and achieve!

Y.E.S’s Supportive Care Model

Our Supportive Model has been woven throughout all our programs and acts as our organisation’s overarching roadmap to delivering personalised, effective, and highly beneficial care to our clients.

The Supportive Model was built on the philosophy that every person deserves to live a full meaningful life. We help people live the lives they choose through respecting and honouring their unique needs.

The Components of Care

To truly provide personalised care, every activity, procedure, and policy must be supportive in nature. At the core of our model is the person. It has to be. The needs of every person are unique!

For people living with a developmental and physical disability there are 5 main areas of needs that are required to provide robust care:

Front line Staff

We understand how important it is to provide individualised disability and home care services to people who trust us to take care of them. We have trained our staff in a way that they excel in meeting every requirement that you might have regarding support needs.
Our team understands your aims and treats them as their own goals and strives to work in a way that they can help you to reach your full potential. Read up on how our Onboarding process and Wellbeing Support Plans.
